COVID-19 Vaccine Appointment Information

Lurie Children's is offering the Pfizer COVID-19 vaccine to individuals 5 years and older. See below for more information on vaccine criteria and appointment availability. 

How to Schedule Your Vaccine Appointment

For Employees, Family Members and Patients (Without an Existing Appointment at the Hospital) 

  • Address225 E. Chicago Avenue, Chicago, 60611 
    • Clinic location: 11th Floor Conference Room
      • Hours: Sunday, January 30 and Sunday, February 6, 9 a.m. – 1 p.m.
      • Appointment required

      • Vaccination Criteria: 
        • Booster (to be administered 5 months after Dose 2) for individuals 12 years and older

For Employees or Patients (With an Existing Appointment at the Hospital)

  • Address225 E. Chicago Avenue, Chicago, 60611 
    • Clinic Location #1: POD 4 (2nd floor)
      • Hours: Wednesdays and Thursdays, 8 a.m.–3:30 p.m. 
      • Employees may walk-in or schedule an appointment through MyChart
      • Walk-ins available for patients or family members with an existing appointment at the hospital 
      • Vaccination Criteria: 
        • Dose 1 or Dose 2 for individuals 5 years and older 
        • Dose 3 (to be administered 28 days after Dose 2) for individuals 5 years and older AND are immunocompromised 
        • Booster (to be administered 5 months after Dose 2) for individuals 12 years and older
    • Clinic Location #2: Onsite Walgreens (3rd floor)
      • Hours: Monday through Friday, 8 a.m.–7 p.m.; Saturday, 9 a.m.– 4 p.m. 
      • Walk-ins available for patients 5 – 11 years old with an existing appointment at the hospital 
      • Vaccination Criteria: 
        • Dose 1 or Dose 2 for individuals to 11 years old only 

For Patients With an Existing Appointment at Clark/Deming

  • Address: 2515 N. Clark Street, Chicago, 60614
    • Clinic Location: Lobby off Clark Street
    • Hours: Mondays and Wednesdays, 8 a.m. – 4 p.m.
    • Walk-in vaccine appointments available for patients with an existing appointment at Clark/Deming:
      • Dose 1 or Dose 2
      • Dose 3 (to be administered 28 days after Dose 2) for individuals 5 years and older AND are immunocompromised
      • Booster (to be administered 5 months after Dose 2) for individuals 12 years and older

For Patients Without an Existing Appointment at Clark/Deming

COVID-19 vaccines will be available to Advanced General Pediatrics and Primary Care patients at Uptown. Please contact our office at 312.227.2610 with any questions or to schedule an appointment.   

Advanced General Pediatrics at Deming

COVID-19 vaccines are available to Advanced General Pediatrics and Primary Care patients at our Clark/Deming Outpatient Center on Mondays and Wednesdays.  Please click here to schedule an appointment at that site.

Frequently Asked Questions

If your child has recently had COVID-19 or has been exposed to someone with COVID-19, you should wait until out of isolation or quarantine before receiving the vaccine. Isolation after being diagnosed with COVID-19 is generally 10 days but may be longer if your child has prolonged symptoms, was hospitalized, or has a weak immune system. Quarantine following exposure to someone with COVID-19 is generally 14 days.

Yes, COVID-19 vaccine can be given at the same time as other vaccines, including influenza. If you recently received another vaccine, you can receive the COVID-19 vaccine as soon as it is available.  

Additional vaccine doses may be required based on your age and medical history:

  • A third dose is recommended for those age 5 years and older with a compromised immune system 28 days after the second dose
  • A booster dose is recommended for those 12 years and older 5 months after the initial series because the first two doses may lose effectiveness over time in adults
  • In those who have had stem cell transplant or CAR-T since getting vaccinated, the COVID-19 vaccine series should be repeated at least 3 months after stem cell transplant or CAR-T 

Lurie Children's is offering additional Pfizer COVID-19 vaccine doses:   

  • Dose 3 (to be administered 28 days after Dose 2)
    • Available for individuals who are immunocompromised AND 5 years and older
  • Booster (to be administered 5 months after Dose 2)
    • Available for all adults age 12 years and older
    • Boosters are NOT yet authorized for anyone younger than 12 years old

No. Lurie Children’s only offers the Pfizer-BioNTech. 

Please bring an ID (if applicable) and your insurance card to your vaccine appointments. There will be no out-of-pocket cost to be vaccinated.

Please wear a short-sleeved shirt or clothing that provides easy access to the upper arm. 

We will schedule your second appointment when you receive your first dose.

You will receive the Centers for Disease Control and Prevention (CDC) vaccination card at your vaccine appointment. If you lose the vaccination card and: 

  1. You have a MyChart account, a copy of your vaccine information will be sent to your account following your appointment.
    • Log into MyChart at mychart.luriechildrens.org. 
    • Open the menu on the left. 
    • Under My Record, click Health Summary.   
    • Click on the Immunizations tab. 
    • Your COVID-19 vaccination record will be listed.

To sign up for a MyChart account, go to mychart.luriechildrens.org.  

  1. You do not have a MyChart account, you can ask for a copy of your vaccine record. To do this, fill out the Authorization for Release of Medical Information form (English | Spanish).
