Transition Team Medical Visit

The Lurie Children's Transition team strives to provide individualized and developmentally appropriate care to prepare youth and their families for the move to adult oriented health care systems. Appointments are available by referral only.

The team can meet with teens and their families to start planning for the transition to adult health care and adult services. The team has clinic twice a month via telehealth.

The Transition Team visits are for: 

  • Teens 14 years old and up
  • Teens and families wanting to be more prepared for the transition to adult health care
  • Teens and families planning for their future needs in school/work, benefits, community programs

During the visit, we will: 

  • Assess transition readiness - enhancing patient knowledge, skills, and responsibility for their health care
  • Connect patients to appropriate community resources, including insurance, benefits, guardianship, post high school resources
  • Create an adult care plan for primary and specialty care
  • Communicate with your primary care and specialist doctors

Please note:

The Transition Team does not provide general pediatric care, handle acute medical issues, or provide chronic disease management. This should be handled by the referring pediatric care provider or subspecialist.

Note: if patient is followed by the primary care section at Lurie Children’s Pediatrics-Uptown, they do not need to be referred because the primary care section provides transition services.
