One of the hardest parts of transitioning may be finding the right doctors to care for you as an adult. Below are a few tips to make your search as easy as possible.

• Start early − a year or two before your 18th birthday
• Be sure to ask your current doctors about their transition policy
• Talk to friends and others who have a similar condition or situation, including disease-specific organizations
• Contact your current insurance provider for suggestions
