Getting Started

To get started, you’ll need to register and schedule your appointment with our practice. You may register by phone by calling 312.227.2800. Please have your insurance information on hand at the time of registration. During registration, you may make your initial appointment.

Your Initial Appointment

• Please come in early for your initial appointment to complete your initial patient forms.
• Bring your insurance card to the appointment. We’ll need to scan a copy.
• After your initial appointment, you may enroll in MyChart to schedule appointments online, message your provider, and review your health information.

What to Expect at Each of Your Child’s Appointments

Download our Well-Visit and Immunization Schedule to review exam, tests, and immunizations we offer throughout your child’s development.

Sending Medical Records to Our Office

You will need to complete a Medical Records request form for us to be able to request your child’s records from a previous provider. Please print and fill out the medical records form and email to your Town & Country Pediatrics office. This form must include a signature to be complete.
