Through Lurie Children's MyChart, we now offer at-home care plans for specific conditions or procedures. 

Care Plans allow you to: 

• Monitor symptoms
• Access condition or procedure-specific education
• Receive important reminders, including medication reminders
• Share health data
• Keep in touch with your care team
