Brain Tumor Board Referral Process

To send a referral to the Lurie Children's Brain Tumor Board, please complete these steps:

• Provide team with a notice of intent to submit a patient’s case for review via email at braintumorboard@luriechildrens.org.
• Download our referral form (click to download​​)
• Send the completed referral form, along with the patient’s demographics and insurance information,​ relevant OR reports, pathology reports, imaging reports and any other clinical notes​ by fax (preferred method) or mail:​
