
Brain Tumor Board Referral Process
To better evaluate and collect information for our weekly Tuesday brain tumor board, please provide the following:
Please include a patient Face sheet (complete with patient name and demographics). Our team likes to review all insurance plans to understand capabilities and plan for possible limitations for second opinions and clinical trial considerations. As such, our coordinators requests that the referring team sends us a copy of the patient’s insurance card too.
Please complete and return a typed BTB referral form, available in Word format (click to download), along with relevant OR reports, pathology reports, imaging reports, and any other pertinent clinical notes. Please note, only typed BTB form will be accepted.

Completed documents and records can be emailed back to braintumorboard@luriechildrens.org
After your patient has been reviewed, a formal “Tumor Board Note” will be placed in Epic that can be viewed through Care Everywhere. We ask that you find formal recommendations there. If you do not have Care Everywhere, please let us know and we will send a separate email.
