Preparing for Your First Day with the Partial Hospitalization Program

When you enter our Outpatient Services at 1440 N. Dayton, you will take the elevators to the 2nd floor and check in with our patient service representatives .

Once on the 2nd floor you’ll need to:

Check in with a service representative and inform them that your child is attending the Partial Hospitalization Program. At 9 a.m., the PHP opens, and staff escorts patients to the unit to begin their day. New patients and parents will wait in the waiting room to meet with a program team member for an overview of the program, staff, and family expectations. Please plan accordingly, as these first-day processes may take up to 1.5 hours. You will also identify the names and organizations to receive information while your child is enrolled in PHP. Each parent/legal guardian and patients 12 years and older must complete and sign a Release of Information Consent Form for contact with individuals outside of Lurie Children's (Emergency Contact, School, Primary Care Physician, Therapist, Psychiatrist). After completing all forms, you will meet with a milieu therapist for a tour of the unit with your child.
