
Preparing for Your First Day with the Partial Hospitalization Program
When you enter our Outpatient Services at 1440 N. Dayton, you will take the elevators to the 2nd floor and check in with our patient service representatives .
Once on the 2nd floor you’ll need to:
- Check in with a point of service representative, and let them know your child is there to attend the Partial Hospitalization Program.
- At 9 a.m., PHP is open and the recreation therapist escorts patients onto the unit to begin their day.
- All new patients and parents will wait in the waiting room to meet with a program team member who will provide an overview of the program, staff and family expectations. You will also identify the names and organizations of those you wish to receive information while your child is enrolled in PHP. A Release Of Information Consent Form must be completed and signed by each parent/legal guardian and all patients 12 years old and older to permit contact with any individual outside of Lurie Children’s (Emergency Contact, School, Primary Care Physician, Therapist, Psychiatrist).
- Once you have completed all of the forms, you will then meet with one of the milieu therapists for you and your child to receive a tour of the unit.
Getting to know your child and how we can best help them is very important to us. Please plan accordingly for the first day because these first day processes may take up to two hours.
