Asthma Mobile Clinic

Our Asthma Mobile Clinic allows families to receive the same level of multifaceted culturally-informed service from Lurie Children’s physicians and allied health professionals in their neighborhood on a mobile unit, rather than at a brick and mortar facility. 

This mobile unit was developed to see patients with all severities of asthma. Avani V. Shah, MD, and Emily R. Simmons, MSN, APRN-NP, PNP, and their multidisciplinary team see patients once a month via the mobile clinic.

Meeting patients in their own communities alleviates many concerns and stressors for families. Lack of transportation to the main hospital location and the cost involved with fuel, parking and/or public transportation can be challenging.

Our goal is to provide families with options leading to improved asthma care for their children.

Residents of the following ZIP Codes are eligible to make an appointment with our mobile clinic: 60618, 60623, 60624, 60634, 60639, 60641, 60644, 60647, 60651 and 60707 by calling 312.227.6730. Lurie Children’s Asthma Mobile Clinic is currently available first Wednesday of every month from 9:00 AM-3:00 PM at Iglesia Evangélica Emanuel located at 5016 W. Armitage Ave., Chicago, IL 60639. Your child must be accompanied by a parent or guardian for the visit.

At an Asthma Mobile Health appointment, your child will be seen by an entire team of asthma experts - a pulmonary provider, asthma nurse, asthma educator and social worker. This is the same care your child would receive at the hospital or at one of our other locations. 

The visits take place at our community partner Iglesia Evangelica Emanuel, with community partners, who allow us to park the mobile unit in their parking lot.  You will be checked in inside the community partner’s building, and some education may take place in the facility., (these facilities could be a local church or community center).

Your child’s visit and exam with the pulmonary provider will take place inside the mobile unit. Appointments are typically about 45 minutes long. We recently added a new mobile spirometer to our unit which gives us the ability to do breathing tests on patients aged 5 and above.  

Yes, mobile health clinic visits are covered by insurance. They are billed the same as a pulmonary visit would be if a patient was scheduled at one of our other Lurie Children’s locations.

In the state of Illinois every child qualifies for AllKids coverage (regardless of income or citizen status). If your child does not have insurance, we will connect you to our Care Advisors who can help start the Medicaid process. Whenever your child is seen by a pulmonary provider, the provider’s clinic note is sent to the Primary Care Physician on file. This allows the Primary Care Physician to stay up to date on your child’s plan of care, and it allows collaboration between your child’s care team.
