Refer a Patient to the Heart Center
We offer several referral pathways to help you connect your patients with the most appropriate cardiac care—whether you’re initiating evaluation for a new concern, coordinating ongoing specialty care in areas such as General Cardiology, Fetal Cardiology, Cardiovascular Surgery, or Heart Transplant, or seeking a second opinion from our experts.

Our Priority Second Opinion Program provides families and referring providers with coordinated second or third opinions in a single outpatient visit. Our interdisciplinary team offers a comprehensive, holistic evaluation to help guide confident care decisions.
Contact our team for help referring a patient to the Heart Center or to speak directly with an on-call cardiac specialist. We're available to help with care coordination, appointment scheduling, and urgent clinical questions.
Our team is available Monday–Friday, 8 a.m.–5 p.m. (excluding holidays) and serves as a single point of contact to ensure timely access to the right Heart Center resources.
