This educational program supports patients and caregivers in understanding dialysis modalities, the path to kidney transplantation, and key issues like growth and nutrition. By fostering early discussions and involving caregivers in decision-making, the GAP Clinic helps reduce stress and improve outcomes.

What to Expect
The clinic conducts most appointments via telemedicine, with in-person options available as well. During each visit, the team educates patients and families about dialysis options, the kidney transplant process, and other related concerns. The expert team ensures families receive all the information needed to make informed decisions. Patients leave with educational materials to continue discussions with their primary nephrologist about the right course of action for them when they reach kidney failure. If needed, families can schedule a follow-up appointment after two years.
