Our team – made up of doctors, nurses, and care coordinators – is committed to providing children with medical complexity with the best comprehensive health care through partnership with their primary care medical home and sub-specialty providers.

Our program serves patients that have complex chronic medical conditions that require multiple specialty physicians, frequent health care utilization (e.g., ED visits or hospitalization), and a need for care coordination. 

Our program goals are:

  • Help navigate the healthcare system and coordinate services between your child’s healthcare providers
  • Support your decisions about your child’s health care goals
  • Partner with your child’s care team if your child is hospitalized
  • Provide assistance with hospital discharge, follow-up care, home equipment, and school needs

Office Hours

Office Hours

Our office is open Monday through Friday from 9am – 4pm. Calls will be returned within one business day.

After Hours/Urgent Care

  • For urgent concerns outside of business hours, please call the hospital operator at 312.227.4000 and ask for the Complex Care Provider On-Call to be paged.  
  • If it is an emergency, please call 911, or go to the closest emergency room.
  • We can also help coordinate more urgent visits with your pediatrician or sub-specialists when necessary.

Interpreter Services

If you prefer to speak a language other than English, please let us know.  We will arrange for an interpreter at the appointment.

Complex Care Clinic Visits

  • We will see you at least every 6 months for a visit and more often when needed.
  • Visits are available in-person and as telemedicine visits.

Before Your Visit

Please make a list of questions you have to discuss with the team. We are interested in understanding what is going well with your child’s care, and what could be improved.

Appointment Day

  • For in-person appointments, please arrive 20 minutes early to allow time for check-in.
  • For telemedicine appointments, we look forward to virtually meeting with you and your child.
