Getting Started

To get started, you’ll need to register and schedule your appointment with our practice. You may register by phone by calling 312.227.1160. Please have your insurance information on hand at the time of registration. During registration, you may make your initial appointment.

Your Initial Appointment

• Please come in early for your initial appointment to complete your initial patient forms.
• Bring your insurance card to the appointment. We’ll need to scan a copy.
• After your initial appointment, you may enroll in MyChart to schedule appointments online, message your provider, and review your health information.

What to Expect at Each of Your Child’s Appointments

Sending Medical Records to Our Office

You will need to complete a Medical Records request form for us to be able to request your child’s records from a previous provider. Please print and fill out the medical records form and email to your Chicago Area Pediatrics office. This form must include a signature to be complete.

New Patient Forms

When you arrive for your first appointment, you will complete our initial forms. If you’d like to get started in advance, you can complete these initial forms:

• Patient Registration Form
• Minors Accompanied by Someone Other Than Legal Guardian (if needed)– The purpose of this form is to identify individuals you would allow to bring your child for visits to Lurie Children's Primary Care - Chicago Area Pediatrics and also to authorize the disclosure of information during visits.
