Established in 2019, Connect Home Visiting Chicago (previously ConnecTeen) is a Maternal Child Health and Early Childhood initiative that refers pregnant people and families with young children to a network of home visiting providers in the Chicagoland area. Connect Home Visiting Chicago uses an enhanced coordinated intake model to serve and support pregnant people and families with young children, including pregnant and parenting students attending Chicago Public Schools (CPS), by connecting them with a network of home visiting programs in their community. After the referral, we provide a warm handoff to one of our partner programs that provide evidence-based home visiting services. Connect Home Visiting Chicago works to raise the visibility of, and access to, home visiting by expanding reach of programs, developing organizational connections, and working directly with schools within Chicago to fully engage youth and support families.
