Established in 2019, Connect Home Visiting Chicago is a Maternal Child Health and Early Childhood initiative that matches pregnant people and families with young children to the best home visiting provider to serve the specific needs of families across Illinois. Connect Home Visiting Chicago partners with nearly 60 home visiting partners across the state and uses a coordinated intake model to provide support to families while they are waiting to get enrolled in home visiting services.
Some benefits home visiting services provide to families include increased parenting education, promotion of child’s development and school readiness in addition to connecting families to care in their communities.
