Nurse Staffing Information

Under the Hospital Report Card Act ("Act") hospitals, including Lurie Children's, are obligated as of January 1, 2004, to share their nursing staff schedules, actual nurse assignment rosters, the methods to determine and adjust staffing levels and staff training information with consumers upon request. Accordingly, the following information is available to the public upon request to the Corporate Compliance Office:

  • Nursing assignment rosters
  • Nursing schedules for each inpatient unit
  • Methodology to determine and adjust nursing staffing levels
  • Information on staff training and orientation

Persons requesting information under the Act will be required to complete a "Request for Information – Illinois Report Card Act" form before the information is disclosed. Once this form has been submitted and signed, Lurie Children's will then provide the appropriate information either verbally or in a format that may be viewed by the requestor. No documentation will be provided to the requestor for their retention purposes.

Upon receipt of the information, the requestor will be asked to sign a "Receipt for Information Provided under the Illinois Hospital Report Card Act" form.

UNDER NO CIRCUMSTANCE SHALL ANY INFORMATION CONTAINING ANY INDIVIDUALLY IDENTIFIABLE PATIENT, EMPLOYEE OR LICENSED PROFESSIONAL BE DISCLOSED TO THE PUBLIC.

Inpatient Hospitalization Requests: For requests related to an individual person's pending or current inpatient hospital stay, information shall be made available within two hours after the request if the request is made during normal business hours (Monday through Friday from 9 a.m. to 5 p.m). If the request is made after normal business hours or on weekends, the requestor should contact the Administrative Coordinator instead of the Corporate Compliance Office, and the information shall be made available within four hours after the request is made.

Other Requests: For requests unrelated to an individual person's pending or current inpatient hospital stay, including requests for historical records, information shall be made available within seven days of receipt of the request and during business hours, Monday through Friday from 9 a.m. to 5 p.m. If the information cannot be provided within seven days of receipt of the request, the requestor will be notified of the reason for the delay and the date by which the records shall be made available.
