Case Management at Ann & Robert H. Lurie Children’s Hospital of Chicago

The Case Management team at Lurie Children’s is staffed by experienced nurse Case Managers and works with families and care teams to coordinate your child’s care, support smooth transitions, and plan for discharge. We help connect families to resources, navigate next steps, and ensure care is safe, timely, and centered on your child’s needs.

Responsibilities of a Nurse Case Manager

While your child is admitted to the hospital, Case Managers work with the interdisciplinary care team to plan for discharge, this may be to home or to another facility. Each discharge plan is tailored to your child’s needs and can include:

  • Medication prior authorizations
  • Transfers to other acute care hospitals, acute inpatient rehabilitation facilities, or inpatient behavioral health units
  • Ordering supplies for home, such as
    • Tube feeding supplies
    • IV Infusion and nursing visits
    • Mobility and Safety Equipment
    • Disposable medical supplies
  • Coordination of home nursing

Case Managers work with any family during the hospital stay, and our services do not require a physician order. If home medical equipment or nursing is needed, a physician order will be required to begin the referral process.

Our team can help to coordinate discharge planning family meetings and will work with you to ensure any homecare company is in-network with your insurance.

Coordination with your Health Insurance

Case Managers and Utilization Management RNs may serve as a resource for families to help you find the right resources to answer questions about your insurance coverage and can connect you to financial assistance, if needed.
