MyChart gives you safe, secure online access to portions of your child's electronic medical record (EMR).

MyChart messaging is not in real-time; allow at least two (2) business days for a response. If your child is having an emergency, call 911. If you think your child may need to be seen, call your child's clinician or 1.800.543.7362 (1.800.KIDS DOC).

Restrictions

A patient must be age 18 or older to become a MyChart account holder. Parents or guardians of children under 18 years of age may become an account holder via a request for proxy access to their child’s account by completing the online registration and submitting an authorization form if required.

Minor patients between the ages of 12 and 17 years (inclusive) are not eligible to become a MyChart account holder, but must provide consent for their parents’ or guardian’s proxy access to their medical information.

Once a minor patient reaches age 18, MyChart access to the proxy account holder will be automatically revoked, and the patient may set up their own account. If the patient is 18 years of age or older and wishes for a parent or guardian to become, or remain, a proxy account holder, the patient and the proxy must complete an authorization form.
