There will be activities for the kids, food, and more.
Photo Consent
A Photo Consent Form must be completed for each person attending. Each person 18 years and older must complete and sign their own form. For children under the age of 18, their parent/legal guardian must complete and sign the photo consent form.

Hand in your completed forms when you arrive at the reunion, or you may email them to us at PedNeurosurgInfo@LurieChildrens.org.
Registration
This is a free event, but advance registration is required.
COVID-19 Protocols
For the safety of our patients, families and employees, Lurie Children's has visitor COVID-19 protocols that you will be required to comply with. Any symptomatic persons are unable to attend.
Parking
Parking is available at the Huron/Superior Garage, located at 222 East Huron Street, Chicago, IL. Use the Huron Street entrance. See more parking details. We will be providing chaser tickets to be used towards the cost of parking.
Checking-In
Once you arrive at the hospital, please go to the 2nd floor concierge desk to sign in. You will need to provide driver license or state ID verification and will be issued visitor ID badges. From there, proceed to the 11th Floor.
