To contact KIDS DOC, call 1.800.KIDS DOC (1.800.543.7362). The KIDS DOC staff can help you schedule or reschedule appointments (option 3) at all Lurie Children's locations, complete regi​stration (option 4) and more. You may also request a first-time appointment online.

Already have an appointment?

To avoid delays on the day of your visit, be sure to register at least 24 hours before your scheduled visit by calling 1.800.KIDS DOC (1.800.543.7362). You may also register online.

If you need to find a doctor or ask a general question, KIDS DOC pediatric nurses are here to help. Call us from 7:30 a.m. to 6 p.m. Monday through Friday and from 8 a.m. to 12 p.m. on Saturday. Call 1.800.KIDS.DOC (1.800.543.7362) and select option 3.

Consent Forms & Documentation

New Appointments: If this is a new appointment, consent forms/documentation must be signed at the visit by a legal guardian.

Return Appointments: If this is a return appointment, the consent forms/documentation will be expired. In order to prevent any interruptions in your child's care, please carefully read, sign and bring the four (4) documents shown below at the time of your child's appointment. These documents can be brought by the adult accompanying your child to the appointment, as long as they are signed by a legal guardian in advance. The documents will be scanned to your child's medical record and will be valid for one (1) year.

Please note:

  • Each child will need their own set of signed consent forms/documents.
  • If these documents are not received at the time of the appointment, we may need to reschedule your child's appointment.

Sign Up for Appointment Reminders

You can now receive text message appointment reminders on your cell phone. Follow the directions below to sign up.

  1. Give your cell phone number to a Lurie Children's staff member when making an appointment or at check-in.
  2. Once we have your cell phone number, text "KIDSDOC" to 622622.

Cancellations & Rescheduling

Call your doctor's office directly at least 24 hours in​ advance. It helps us, and it helps other families receive ti​mely​ care​. If you do not have your doctor's direct phone number, call 1.800.KIDS DOC (1.800.543.7362, option 3) to cancel or reschedule an appointment.

Convenient Care

With extended hours and walk-in service, Convenient Care offers same-day medical services for the unexpected illnesses on the 8th floor of our Lincoln Park Outpatient Center. Pediatricians provide treatment for non​-emergencies or non-life-threatening illnesses and injuries.
