Lurie Children’s is now offering virtual outpatient appointments via real-time video or phone. Some appointments will still require you to be seen in person, but your physician and care team will let you know if a telemedicine appointment is available. Learn more about our Telemedicine Program

With extended hours and walk-in service, Immediate Care offers same-day medical services for non​-emergencies or non-life-threatening illnesses and injuries at our Outpatient Center in Lincoln Park and Outpatient Center in Northbrook. Learn more about our Immediate Care Services

If your child has been diagnosed with a serious medical condition or illness, consider getting a second opinion from the pediatric specialists at Lurie Children's. A second opinion from Lurie Children's can offer peace of mind that your child is receiving appropriate medical care.

New Appointments: If this is a new appointment, consent forms/documentation must be signed at the visit by a legal guardian.

Return Appointments: If this is a return appointment, the consent forms/documentation will be expired. In order to prevent any interruptions in your child's care, please carefully read, sign and bring the four (4) documents shown below at the time of your child's appointment. These documents can be brought by the adult accompanying your child to the appointment, as long as they are signed by a legal guardian in advance. The documents will be scanned to your child's medical record and will be valid for one (1) year.

Please note:

  • Each child will need their own set of signed consent forms/documents.
  • If these documents are not received at the time of the appointment, we may need to reschedule your child's appointment.
