Our prog​ram participates in the National Residency Matching Program. We accept one fellow each year.

Interested applicants should submit the AMSSM Primary Care Sports Medicine application including supporting materials:

  • Three letters of recommendation from physicians you have worked with in a clinical setting, including one from program director
  • Copy of medical school diploma, certificates from previous training, curriculum vitae, copy of current state medical licenses

In addition, please submit:

  • Medical school dean’s letter and transcripts
  • Copies of any publications
  • Copies of USMLE scores and specialty board scores (if taken)


Applicants must have completed a residency in pediatrics, family medicine, med/peds, physical medicine and rehabilitation, internal medicine or emergency medicine.  Because our hospital’s mission is to care for children and adolescents, we will give priority to candidates with a background in pediatrics.  Applicants must be board certified or board- eligible and be eligible for a medical license in the state of Illinois.
