Important Information About the Plans We Accept
Determining health care benefits and coverage is very complicated. It is best to verify your coverage by reviewing your plan’s benefit book or contacting your plan’s customer service department for details on:
- Member’s financial responsibility: deductible, co-pay and co-insurance
- Plan’s policy for referrals, prior-authorization or precertification
- Covered and non-covered services
- Network status of providers
Insurance companies may utilize specialty networks for some services, including but not limited to: mental health, rehabilitation (therapy), eye and/or dental care. Please be sure to call your insurance company before making an appointment to confirm that both the hospital and provider are covered by your plan.

Help Applying for Medicaid and Exchange Plans
Lurie Children’s offers free assistance with applying for Medicaid, healthcare exchange plans or food stamps (SNAP) with Patient Innovation Center (PIC) Navigators. They are available to help you enroll in the health plan that is right for you and your family. These trained, state-licensed professionals can help you understand coverage, answer questions you have and guide you through any issues with your insurance. Learn more.
